Maintenance Requests are handled during business hours only (Monday-Friday 9AM-4PM).  Maintenance hours are available outside of these hours at tenant’s cost.  Call for emergencies: 866-356-5408, Ext. #5.

Please use the following form to submit any maintenance issues you might be experiencing. Once the form is submitted you will receive a telephone call or written confirmation that includes a service request number for tracking purposes. A service representative will review your request and contact you shortly to discuss the problem.

By signing and submitting this form I am requesting Maintenance Service Request for the inspection and resolution of problem issue(s) listed above in accordance with the rental/lease agreement.  I understand in accordance with the rental/lease agreement that I am, as tenant, responsible for the care and cleanliness of the property.  I understand this includes, but it is not limited to:  cleaning windows, window sills, entry ways, carpets, sinks, toilets, tubs, & appliances on a regular and consistent basis.  Additional property care items include:  lawn care, changing HVAC air filter monthly, keeping the HVAC on at all times set at a reasonable temperature, turning off running water, removing standing water, temporarily treating any problem issue and reporting all maintenance issue(s)/concerns in writing within 24 hours of appearance.

Maintenance on the rental property is provided in accordance with the terms established in the rental/lease agreement.  Landlord schedules inspections/treatment of all maintenance issues within 24-72 hours upon receipt.  Tenant is responsible for meeting maintenance inspector.  Tenant accepts the inspection results, problem resolution and cause determination as determined and conducted by the maintenance contractor as final.  Liability for the payment for the maintenance expense is determined by the cause of the problem issue(s); and payment for the maintenance resolution may be collected from tenant if problem issue(s) is determined to be a result of tenant’s neglect or cause.  Tenant agrees to pay for all maintenance determined to be caused by tenant, tenant’s family, friends, or guests; and tenant agrees to pay for all maintenance costs associated with a false maintenance request, if no problem issue(s) is discovered/determined.
